The form is part of the project intake; it does not constitute a quotation or compliance approval.
Useful attachments include reflected ceiling plans, room schedules, lighting calculations, control schematics and an equipment schedule showing the current basis of design. Note whether the inquiry concerns early selection, a sample, a tender alternates review, a released order or a warranty question. This context lets the project desk route the request without assuming every issue is a product-selection question.
For photometric review, identify mounting height, work-plane height, reflectance assumptions and required output format. For controls coordination, list the protocol, sensor scope, emergency behavior and commissioning owner. For delivery planning, provide quantities by area and the sequence in which the installer expects material.
A response may require regional product confirmation. A family shown in one market may use another driver, certification set or ordering code elsewhere. Final suitability follows the written quotation, approved submittal and project-specific engineering review. Do not submit medical, financial or other sensitive personal information.
